Obstructive Azoospermia
40% of azoospermic males suffer from the obstructive condition. Spermatogenesis tends to be normal in obstructive azoospermia, but the reproductive tracts of both testicles get obstructed. Due to this, there are no observable sperm in the semen.

Non-Obstructive Azoospermia
Being the most prevalent form of azoospermia, it affects around 60% of azoospermic males. Severe problems in spermatogenesis, which are commonly caused by testicular failure or malfunction, are the most common cause of nonobstructive azoospermia. It may also be caused by pituitary or hypothalamic dysfunction.

Azoospermia Causes
Three categories—pre-testicular, testicular, and post-testicular—can be used to group potential azoospermia causes.

Pre-testicular
Pre-testicular azoospermia refers to endocrine abnormalities that have an adverse effect on spermatogenesis. They are caused by anomalies in the pituitary gland and/or hypothalamus, both of which are found in the brain. This imbalance of hormones prevents otherwise normal testicles from being triggered to produce sperm. It occurs due to low testosterone (hypogonadism), inadequate diet, and the use of specific medications.

Testicular
Testicular etiologies are caused by inherent disorders in the testes’ spermatogenesis. It implies that there is an issue in sperm generation within the testicles themselves. Sperm production might be absent completely or may get interrupted at any point in the early or late stages of sperm maturation. These problems can be either congenital, i.e., existing from birth, or may be acquired at any point later in life.

Post-testicular
Post-testicular cause of azoospermia implies the ductal system being blocked anywhere in the male reproductive tract. Even though the testicles are producing sperm, there is no ejaculation because of an obstruction in the genital tract, which blocks the reproductive tracts of both testicles. As a result, the semen cannot comprise any detectable sperm.
Diagnosing Azoospermia

Semen Analysis
Spermatogenic failure or blockage between the testes and the seminal vesicles is observed in azoospermic patients with normal ejaculate volume. Ejaculatory dysfunction or ejaculatory duct blockage could be seen in azoospermia patients with normal-sized testicles and poor semen volume.

Hormonal Evaluation
A thorough endocrinological evaluation of hormonal profiles such as testosterone level (total and free) and follicle-stimulating hormone levels should be collected. In individuals with obstructive azoospermia, the FSH level is often low or normal. On the other hand, FSH levels are usually high in nonobstructive azoospermia, particularly when the testes are smaller than normal size.

Imaging (Scrotal Ultrasonography)
Scrotal ultrasonography procedure helps in measuring the size of the testis more precisely and can identify lesions, cysts, and spermatoceles that would not be visible with other procedures. Moreover, it can assist in distinguishing between obstructive and nonobstructive azoospermia.
